What 24-Hour Home Care Looks Like in Middletown

A River Port That Became a College Town

Middletown was incorporated in 1650 under the name Mattabeseck, was renamed in 1653 and became a city in 1784. The city describes itself as originally a busy sailing port, then an industrial center, and today the downtown is a college town for Wesleyan University. All of it, from downtown out to Westfield, Newfield and Maromas, is inside the area we cover.

Home From Middlesex Hospital

Many families first ask about round-the-clock care when a hospital stay ends and the person comes home needing more help than before. We are not connected to Middlesex Hospital, and the hospital arranges the follow-up care. What we add is the hours at home: a caregiver awake and present at every one of them, in a plan that can change as the need does.

Every Hour Counted, Not Just Watched

Round-the-clock care covers more than keeping an eye on someone. It includes help with hygiene, with drinking and eating, with moving safely at night, and company when the house is quiet. Whether the home is an apartment near Main Street or a house in Newfield, it is the same continuous presence.

Our only office is at 1436 Berlin Turnpike in Berlin, 9.9 miles from Middletown by road, and we have no Middletown address. The caregiver comes to the house, while your client manager and the schedule stay with the office, a phone call away.

Getting Started Is Simple

A Clear, Guided Process from Initial Call to Caregiver Match

  1. Reach out by phone or by filling out our online form, and THREE things come back to you. A FREE foundational Plan of Care tailored to you and your loved one, along with potential caregiver(s) profile(s) near you, and a list of questions to ask home care agencies.

  2. Our goal is to create the most effective Plan of Care for your loved one. One of our Client Managers will reach out to schedule an in-home visit. This allows us to get to know you and your loved one personally, understand their needs, and assess the home for safety. We'll note any existing adaptive equipment and flag any adjustments that could better support their independence at home.

  3. Based on everything we have learned about your loved one, we will send you a couple of pre-selected caregiver profiles that we believe are a good match for your loved one's needs, preferences, hobbies, personality, etc. Once you have chosen a caregiver you feel good about, you have the option to meet them before services begin.

  4. On the first day of services, our Field Supervisor will be on-site to ensure the Plan of Care is fully understood and followed. Clients are encouraged to ask questions, and we will be there to ease the transition every step of the way. From there, our Field Supervisor will continue with monthly in-home visits to ensure quality care is maintained.

Why You Need a Plan of Care for Your Loved One?

A Plan of Care tells you exactly what level of support your loved one needs without committing to anything. We put it together because every family making this decision deserves clarity before they commit.

Tailored specifically to your loved one — their routines, their preferences, their history, their situation. Not a template. Not a generic checklist. A plan built for one person — yours.

You own the plan — always. You walk away with it regardless of what you decide next.
